5Promise of Statutory Representatives

On behalf of the applicant named above we hereby apply for membership in the World Minifootball Federation ("WMF"), registered in the Czech Republic under identification number 751 28 110.

We accept the WMF Statutes, rules and regulations and promise to observe them. We will always act honestly and fairly towards other members of the WMF.

We agree that all information provided in this application may be used for WMF needs in accordance with the Personal Data Protection Act of the Czech Republic and the European Union, where the WMF is registered.
